Addiction is a medical condition in which the brain's reward and control systems have adapted to a substance or behaviour, so that stopping takes more than willpower. Dr Vohra's treats alcohol, cannabis, opioid, tobacco and prescription-drug dependence and behavioural addictions as an outpatient clinic: psychiatric and medical assessment, safely managed withdrawal, anti-craving medication, therapy for relapse prevention, and family sessions. Dr. Aditya Vohra leads this work at East Patel Nagar, New Delhi, with online follow-ups.

## Use, harmful use, dependence

ICD-11 distinguishes a single episode of harmful use, a harmful pattern of use, and dependence (for alcohol, 6C40.2). Dependence is marked by impaired control, use taking priority over other activities, and physiological features such as tolerance and withdrawal. The National Mental Health Survey (2015-16) found that about one in twenty Indian adults had an alcohol use disorder and one in five a tobacco use disorder; treatment-seeking is rare because of shame, which the clinic's confidentiality is designed to address.

## How is addiction treated at Dr Vohra's?

Assessment: what is used, how much, for how long, previous attempts to stop, physical health (liver, nutrition, infections where relevant), and co-occurring depression, anxiety, ADHD or psychosis, which are present in a large share of people with addiction and must be treated too.

Withdrawal: planned and medically managed; alcohol and benzodiazepine withdrawal can be dangerous and are never left unsupervised. Anti-craving medication: naltrexone or acamprosate for alcohol; buprenorphine-based treatment for opioids through licensed channels; varenicline, bupropion or nicotine replacement for tobacco.

Therapy: motivational work first, then CBT-based relapse prevention (triggers, cravings, refusal skills, lifestyle), with family sessions so relatives know how to help. Follow-ups are weekly at first; recovery is measured over months, and a lapse is treated as information, not failure.

## When outpatient care is not enough

Severe withdrawal risk, an unsafe home environment, repeated relapse despite outpatient care, or serious medical or psychiatric complications call for admission. The clinic refers to a hospital or licensed centre and resumes care after discharge.
